Initial visa medical examination is strictly by appointment. Please submit an online registration to set an appointment.
The following applicants should contact SLEC at 521-0020 for their examination/testing schedule:
- Applicants referred by other screening facilities
- Applicants for DNA testing
Applicants who have been scheduled for additional examinations (e.g. Sputum collection) and/or DNA Testing must come on their designated examination date and time.
REQUIRED DOCUMENTS
- Original passport or a copy of passport certified by a Canadian Visa Office
- National ID Card (an original Birth Certificate is an acceptable national ID document for minors under the age of consent only.)
- Refugee travel document
- Red Cross travel document
- UN laissez-passer
- Seaman's Book
- Organization of American States travel document
- Refugee Protection Claimant Document
- Birth certificate, with School/College/University ID Please ensure that the birth certificate is readable.
- Printed Online Registration Email Confirmation
- 2 PHOTOCOPIES of passport/birth certificate (see format)
- 2 pieces recent passport photos (white background)
- Medical Instruction from the Embassy (Embassy Form 1017), if available
For Express Entry Applicants:
CIC IMM 5791 form (Invitation to Apply under Express Entry) - Photocopy of Embassy Form 1017 or CIC IMM 5791 form (Invitation to Apply under Express Entry)
Minors (15 years old and below) must be accompanied by their parent or by a legal guardian who has thorough knowledge of the child's medical history.
The parent or legal guardian must present a government issued ID (original and 2 photocopies)
In case the companion is not the legal guardian, the following documents must be presented:
- Letter of Authorization or Special power of attorney stating that the companion is authorized to sign on behalf of the parent
- 2 photocopies of the Parent's government issued ID
- Authorized companion's government issued ID, original and 2 photocopies
The client must submit a passport or other identification document acceptable for the Canadian Immigration medical examination. All documents must be valid (not expired). Approved identification documents include the following:

For children/students who do not have the identity documents listed above, the following may be presented:
IMPORTANT!
The accompanying parent/guardian/close relative MUST present one of the identity documents listed above.
Whenever applicable, please bring the following:
- Old chest x-ray films not less than 3 months prior to the medical examination
- Medical certificate regarding a previously-treated or currently managed clinically significant illness The medical certificate should contain the following information:
- Medical Treatment
- name of medication (brand and/or generic name)
- dosage
- duration and dates of treatment
- surgical procedure done
- complete final diagnosis
- outcome of treatment or treatment plans and prognosis, if available
- Vaccination Record
must contain the following details:- personal copy (e.g. baby book)or copy of medical chart indicating received vaccines
- complete date of receipt of vaccination
- must include signature (license number, if applicable) of doctor or health worker who administered vaccine

Medical Examination Fees

ANNOUNCEMENT
INCREASE IN CANADA MEDICAL FEES BEGINNING JULY 1, 2018
Beginning July 1, 2018, there will be an increase in the visa medical fees for Canada visa applicants. Please see table of fees below:

0 to 4 years old | PhP 2,950.00 |
5 to 10 years old | PhP 3,800.00 |
11 to 14 years old | PhP 5,600.00 |
15 years old and up | PhP 11,550.00 * |
Fees are inclusive of TB Testing and TB Treatment for applicants. Additional Non-TB tests are not covered by the new fees and will be charged accordingly.
* Beginning May 1, 2019, IRCC has added a new requirement for serum creatinine blood test for all visa applicants ages 15 years old and above. An additional fee of Php 625.00 will be charged for the test on top of the medical exam package.
St. Luke's Medical Center Extension Clinic accepts cash and credit card payments at the clinic cashier. Applicants who register online may also pay via Security Bank Bills Payment Facility.

REMINDERS:
- Applicants are advised to take their meals prior to coming over for their medical examination since food is not allowed within the premises and all the initial blood and urine tests do not require a fasting state. There are restaurants near St. Luke's Medical Center Extension Clinic and drinking fountains are available within the building.
- Females within the reproductive age group are advised not to schedule their medical examination during their menstrual period. Otherwise, they will be required to come back one week after the last day of their menstruation.
Minors (15 years old and below) must be accompanied by their parent or by a legal guardian who has thorough knowledge of the child's medical history.

- The medical report, and chest x-ray are directly submitted by St. Luke's Medical Center Extension Clinic via eMedical.
