Listed below are the INITIAL tests required for your visa medical examination. Significant health conditions identified during initial medical examination may require further clinical investigation, based on the visa medical examination guidelines set by the Australian Government, Department of Immigration and Border Protection.

1. PERMANENT Visa Applicants

5-10 years old	Medical History and Physical Examination, urinalysis, Tuberculin Skin Test(TST)/ IGRA
11-14 years old	Medical History and Physical Examination, urinalysis, chest x-ray (CXR)
15 y/o and above	Medical History and Physical Examination, Serum Creatinine, CXR, HIV

• Permanent visa applicants 2-10 years old are required to undertake either a Tuberculin Skin Test (TST) OR a TB Blood Test (Interferon Gamma Release Assay or IGRA)

• Tuberculin Skin Test (TST) requires 2 clinic visits. Second visit is between 48 and 72 hours from TST placement.

To avoid TST from becoming invalidated, please be at the clinic at least an hour before the schedule written on your TST return slip.

• The TB Blood Test (Interferon Gamma Release Assay or IGRA) will only require 1 clinic visit if results are negative.

• Chest X-ray will be required for children 10 years old and below if they have a positive TST/IGRA result or have exposure to TB Cases or a history of TB disease

2. TEMPORARY Visa Applicants

Chest X-ray will also be required for children 10 years old and below if they have had exposure to TB Cases or history of TB disease.

3. STUDENT or WORKER (Hospital or Healthcare)

Routine medical examination requirements for adults PLUS IGRA*, HIV*, HBsAg*, and Anti-HCV* (regardless of length of stay)

* to be charged separately as tests are not part of the medical examination package fee

4. WORKER (457 Visa Applicant): Non-Hospital or non-Healthcare

Form 160 (Chest X-ray Only)

Parents are strongly encouraged to consider immunizing their children against:

Rotavirus vaccination is encouraged in infants between two and eight months of age.

The Australian embassy strongly encourages applicants especially children, to complete their immunization before travel to Australia.

Immunization is required for school enrolment as well as settlement-related purposes including family benefits.

TB screening and treatment are included in the fees, while all non-TB tests will be charged accordingly.

REMINDERS:

1. Applicants are advised to take their meals prior to coming over for their medical examination since food is not allowed within the premises and all the initial blood and urine tests do not require a fasting state. There are restaurants near St. Luke's Medical Center Extension Clinic and drinking fountains are available within the building.

2. Females within the reproductive age group are advised not to schedule their medical examination during their menstrual period. Otherwise, they will be required to come back one week after the last day of their menstruation.

3. Minors (17 years old and below) must be accompanied by their parent or by a legal guardian who has thorough knowledge of the child's medical history.

The parent or legal guardian must present a government issued ID (original and 2 photocopies)

In case the companion is not the legal guardian, the following documents must be presented:

• Letter of Authorization or Special power of attorney stating that the companion is authorized to sign on behalf of the parent

• 2 photocopies of the Parent's government issued ID

• Authorized companion's government issued ID, original and 2 photocopies

Note:

• Medical results and documents are completed according to the physical examination, x-ray and laboratory results, which will be conducted and processed in accordance with the guidelines set by the Australia Government - Department of Immigration and Citizenship.

• For eMedical applications, medical reports and x-ray (DICOM image) are electronically submitted online.

DNA SPECIMEN COLLECTION

1. By appointment only

2. Every 1st and 3rd Wednesday of the month

3. An Australian Embassy representative must be present to witness the specimen collection.

4. Fees

• Administrative Fee (including Collection):

PhP 3,675.00 per case

• DNA Testing c/o St. Luke's Medical Center:

PhP 27,134.00 per person

• DNA Testing c/o Accredited DNA Laboratory in Australia:

5. To set an appointment for DNA testing for Australia, kindly call us thru (+632) 8526-0208 or send an email to dna.aupanel@slec.ph and include the following details:

1. Name of applicant / donor with his/her contact number.

• if the applicant is a minor, kindly provide the name of his/her authorized guardian and contact number

2. Kindly indicate if all donors will be collected at St. Luke's Medical Center Extension Clinic

• If one of the donors will be collected in Australia, kindly provide the name of the accredited NATA laboratory

3. Invitation letter / DNA request letter from the Department of Home Affairs

4. Scanned copy of the donors' valid passport, if unavailable, valid government issued ID and Child's Birth Certificate
