Medical Examination for Canada Visa Applicants
The client must submit a passport or other identification document acceptable for the Canadian Immigration medical examination. All documents must be valid (not expired). Approved identification documents include the following:

Original passport or a copy of passport certified by a Canadian Visa Office
National ID Card (an original Birth Certificate is an acceptable national ID document for minors under the age of consent only.)
Refugee travel document
Red Cross travel document
UN laissez-passer
Seaman's Book
Organization of American States travel document
Refugee Protection Claimant Document
For children/students who do not have the identity documents listed above, the following may be presented:

The accompanying parent/guardian/close relative MUST present one of the identity documents listed above.
Birth certificate, with School/College/University ID
Please ensure that the birth certificate is readable

2 PHOTOCOPIES of passport/birth certificate (see format)
2 pieces recent passport photos (white background)
Medical Instruction from the Embassy (Embassy Form 1017), if available
Photocopy of Embassy Form 1017 or CIC IMM 5791 form (Invitation to Apply under Express Entry)
Minors (15 years old and below) must be accompanied by their parent or by a legal guardian who has thorough knowledge of the child's medical history.
Letter of Authorization or Special power of attorney stating that the companion is authorized to sign on behalf of the parent
2 photocopies of the Parent's government issued ID
Authorized companion's government issued ID, original and 2 photocopies
For Express Entry Applicants:
CIC IMM 5791 form (Invitation to Apply under Express Entry)
The parent or legal guardian must present a government issued ID (original and 2 photocopies)
In case the companion is not the legal guardian, the following documents must be presented:
Whenever applicable, please bring the following:
Old chest x-ray films not less than 3 months prior to the medical examination
Original COVID-19 Vaccination card and photocopy
Medical certificate regarding a previously-treated or currently managed clinically significant illness
Medical Treatment

dosage
duration and dates of treatment
surgical procedure done
complete final diagnosis
outcome of treatment or treatment plans and prognosis, if available
Vaccination Record
personal copy (e.g. baby book)or copy of medical chart indicating received vaccines
complete date of receipt of vaccination
must include signature (license number, if applicable) of doctor or health worker who administered vaccine

COVID-19 Screening and Vaccination
Applicants with signs and symptoms of possible COVID 19 will not be allowed to proceed with the medical examination and will need complete the recommended isolation period.
All visa applicants who are close contacts of suspected COVID-19 cases will also be required to complete the quarantine period prior to doing their medical examination.
Applicants are asked to bring the original and clear photocopy of their COVID-19 vaccination record.

Fees are inclusive of TB Testing and TB Treatment for applicants. Additional Non-TB tests are not covered by the new fees and will be charged accordingly.
REMINDERS:
Applicants are advised to take their meals prior to coming over for their medical examination since food is not allowed within the premises and all the initial blood and urine tests do not require a fasting state. There are restaurants near St. Luke's Medical Center Extension Clinic and drinking fountains are available within the building.

Note:
Medical results and documents will be released not on a first-come first-served basis but depending on the physical examination, x-ray and laboratory results, which will be conducted and processed in accordance with the guidelines set by the Canadian Embassy - Health Management Branch.
The medical report, and chest x-ray are directly submitted by St. Luke's Medical Center Extension Clinic via eMedical.
