Medical Examination for New Zealand Visa Applicants

  3. If not available, the following identification documents may be presented:

    • national ID (SSS, postal ID, voter's ID, PRC ID, driver's license) or

    • school ID with birth certificate

  4. Two (2) PHOTOCOPIES of valid passport/ national ID/ school ID and birth certificate

  5. 6 Passport size photos (white or blue background)

  6. Letter/Medical instruction for Medical (if available)

  7. Minors (15 years old and below) must be accompanied by their parent or by a legal guardian who has thorough knowledge of the child's medical history.

  8. The parent or legal guardian must present a government issued ID (original and 2 photocopies)

    In case the companion is not the legal guardian, the following documents must be presented:

    • Letter of Authorization or Special power of attorney stating that the companion is authorized to sign on behalf of the parent

    • 2 photocopies of the Parent's government issued ID

    • Authorized companion's government issued ID, original and 2 photocopies

Whenever applicable, please bring the following:

  • Old chest x-ray films not less than 3 months prior to the medical examination

  • Original COVID-19 Vaccination card and photocopy

  • Medical certificate regarding a previously-treated or currently managed clinically significant illness

  • The medical certificate should contain the following information:

    • Medical Treatment

      • name of medication (brand and/or generic name)

      • dosage

      • duration and dates of treatmen

    • surgical procedure done

    • complete final diagnosis

    • outcome of treatment or treatment plans and prognosis, if available

COVID-19 Screening and Vaccination
  • All visa applicants are required to present a negative RT PCR test done within 3 days of their scheduled medical examination. Counting of days start with the swabbing and not the date of release of the results.

  • Only Nucleic Acid Amplification Tests such as RT PCR using nasopharyngeal swabs are acceptable. Rapid Antigen Tests or saliva tests will not be accepted.

  • Applicants with signs and symptoms of possible COVID 19 will not be allowed to proceed with the medical examination and will need to repeat their RT PCR test and complete the recommended isolation period (10 days for mild/moderate cases, 21 days for severe/critical cases).

  • All visa applicants who are close contacts of suspected COVID-19 cases will also be required to do RT PCR and complete a quarantine period of 7-14 days prior to doing their medical examination.

    • If fully vaccinated - quarantine for 7 days

    • If partially vaccinated or unvaccinated - quarantine for 14 days

Specialty consultations available within the clinic are performed free of charge. Referrals from other clinics will be charged accordingly.

St. Luke's Medical Center Extension Clinic accepts cash and credit card payments at the clinic cashier. Applicants who register online may also pay via Security Bank Bills Payment Facility.

REMINDERS:

  1. Applicants are advised to take their meals prior to coming over for their medical examination since food is not allowed within the premises and all the initial blood and urine tests do not require a fasting state. There are restaurants near St. Luke's Medical Center Extension Clinic and drinking fountains are available within the building.

  2. Females within the reproductive age group are advised not to schedule their medical examination during their menstrual period. Otherwise, they will be required to come back one week after the last day of their menstruation.

  3. Otherwise, they will be required to come back one week after the last day of their menstruation.

  4. Minors (15 years old and below) must be accompanied by their parent or by a legal guardian who has thorough knowledge of the child's medical history.

  • Medical results and will be released according to the physical examination, x-ray and laboratory results, which will be conducted and processed in accordance with the guidelines set by Immigration New Zealand.

  • The medical report and chest x-ray are submitted by St. Luke's Medical Center Extension Clinic released to the applicant, to be directly submitted to Immigration New Zealand.
