Medical Examination for Falkland

REMINDER:
Fee is NOT inclusive of TB Testing and TB Treatment for applicants if furthered.
If furthered for sputum exams:

  • Sputum Examinations - Php 10,300.00
  • Repeat chest x-ray (3 months from the initial cxr or as requested) - Php 919.00 (per view)
Falkland Dental Examination Form must be completed by the visa applicant’s attending dentist

  • Medical Examination Appointment Confirmation Email from SLEC (printed or digital copy)

  • Proof of payment if paid online (printout or digital copy)

  • Valid original passport and one (1) photocopy of biographic data page

  • Six (6) Philippine passport sized photos

  • Falkland Island Medical Examination Form

    • For minors (15 years old and below):

  • Original and one (1) photocopy of legal guardian's government issued ID

  • In case the companion is not the legal guardian, the following documents must be presented:

    • Letter of Authorization stating that the companion is authorized to sign on behalf of the parent

    • one (1) photocopy of the Parent's government issued ID

    • Authorized companion's government issued ID, original and one (1) photocopy

Falkland Island Dental Examination Form must be completed by the visa applicant’s attending dentist

TB screening and treatment are included in the fees, while all non-TB test will be charged accordingly.
Please be guided accordingly.
Specialty consultations available within the clinic are performed free of charge. Referrals to other clinics will be charged accordingly.
Additional test deemed necessary in the course of physical exam are charged accordingly.
